Do you have a history of vascular disease in your family?




Do you have swelling in your legs, feet, or ankles?




Do you sit for prolonged periods of time during a typical day?




Do you stand for prolonged periods of time during a typical day?




Do you often feel fatigued during a typical day?




Do you smoke 5 or more cigarettes per day?




Do you exercise less than thirty minutes per day, three times per week?




Do you have Type 1 or Type 2 diabetes?




Do you have a high blood pressure? The definition of high blood pressure varies based on age as well as other factors, but in general, high blood pressure is defined as having a systolic (when heart is pumping) blood pressure of 120 or higher, and a diastolic (when heart is at rest) blood pressure of 80 or higher.




Do you eat meals high in saturated fat three or more times per week?




Do you have high cholesterol? Cholesterol of 200 mg/dL or greater is considered high.




Do you have a high Body Mass Index (BMI)? A BMI of 25 or greater is considered high.
